HOSPICE & PALLATIVE CARE PROVIDERS LOS ANGELES & NEAR BY COMMUNITIES

We are one of the most experience hospices care and palliative care providers serving patients across Southern California. Our hospice care team focuses on helping our patients live life to its fullest with dignity, care and respect. We develop a comprehensive care plan uniquely designed to meet each individual person’s culture, values, and beliefs. Our hospice care team is available 24 hours a day, seven day a week to help support our patients and their loved ones every step of the way.


Registered Nurses & LVN's

Our nurses evaluate and assess patient medical needs and render care to patients, instruction and teaching in accordance in with the Plan of care, in accordance with the Interdiscipinary Team.

Speech Therapy, Physical Therapy, & Occupational Therapy

Our therapists will assess speech and/or physical needs and activities that can be performed by the patient.

Medical Social Worker

Our social worker will assess the psychological needs of the patient and family and provide support with communitiy activities.

Certified Home Health Aides

Our home health aides will assist you on your activities of daily living such as grooming, bathing, hygiene, eating, and light housekeeping under the supervision of a Registered Nurse.

Registered Dietician

Our dietician will assess the patients dietary needs. They also specialize in meal planning and special diets (Diabetic, Low Sodium, Renal diet, etc).

Durable Medical Equipment

Durable medical equipment will be provided to the patient's as needed such as canes, walkers, wheelchairs, etc. as per physicians orders.

What is Hospice?

Hospice is a special concept of care focusing on symptoms management (ie pain relief) and comfort measures, and to provide support to the patient and family to be comfortable, emotionally and physically.

Who Pays for Hospice?

The cost associated with the hospice care is generally covered under the Medical/Medicare Hospice Benefit and Health Management Organization (HMO) or private insurances.
